Reservation and Deposit Policy for Gastroscopies and Colonoscopies
This document governs the reservation and deposit policy for endoscopic procedures at the Mazan Surgery Clinic in Katowice (Zbigniew Mazan, Mazan Surgery Clinic in Katowice, ul. Brzozowa 54, Tax Identification Number: 6252029012 – hereinafter referred to as the Clinic).
Reservations for procedures are made by making an appointment at the Clinic’s reception desk, either in person or by phone.
Reservations for gastroscopies and colonoscopies require a deposit of PLN 200 to be valid. The deposit must be paid no later than 4 calendar days (i.e., 96 hours) before the scheduled appointment. Failure to pay the deposit will result in the reservation being automatically canceled.
The patient may cancel the reservation without any financial consequences no later than 4 calendar days (i.e., 96 hours) before the scheduled date. Cancellations can be submitted via email (to sekretariat@klinika-mazan.pl) or by phone via text message (to 691 476 095).
The deposit should be paid to the Clinic’s account:

In the transfer description, please specify: “deposit for endoscopic procedure” and, if payment is made from an account other than the patient’s, also include the patient’s name and surname.
- The deposit paid has the following meaning and will be settled as follows:
If the Patient avails of the treatment on the scheduled date, the deposit amount will be applied towards the treatment price charged in accordance with the Clinic’s current price list.
If the Patient fails to avail of the treatment on the scheduled date (does not show up at the Clinic) or cancels the reservation less than 4 days (i.e., 96 hours) before the procedure, the deposit will be forfeited and retained by the Clinic.
If the reservation is canceled in accordance with point 1. 3 above (i.e., no later than 4 calendar days (i.e., 96 hours) before the scheduled date), the deposit will be refunded to the Patient in full.
If the procedure cannot be performed on the scheduled date due to reasons attributable to the Clinic, at the Patient’s discretion, the deposit amount may be transferred to cover the procedure on another date agreed upon by the Parties. If no alternative date is agreed, the deposit will be refunded to the Patient in the amount of twice the amount paid.

This document governs the reservation and deposit policy for endoscopic examinations at the Mazan Surgery Clinic in Katowice (Zbigniew Mazan, Mazan Surgery Clinic in Katowice, ul. Brzozowa 54, Tax Identification Number: 6252029012 – hereinafter referred to as the Clinic).
Reservations for procedures can be made by making an appointment at the Clinic’s reception desk, either in person or by phone.
Reservations for endoscopic capsule examinations require a PLN 500 deposit to be valid. The deposit must be paid no later than 5 business days (120 hours) before the scheduled appointment. Failure to pay the deposit will result in the reservation being automatically canceled.
Reservations may be canceled by the patient no later than 5 business days before the scheduled appointment. Cancellations can be submitted via email (to sekretariat@klinika-mazan.pl) or by phone (text message) at 691 476 095.

In the absence of the Clinic, the deposit amount will be transferred towards the procedure at another date agreed upon by the Parties.
