Rules for Booking and Paying a Deposit for Gastroscopy and Colonoscopy Procedures

This document sets out the rules for booking and paying a deposit for endoscopic procedures applicable at Klinika Chirurgii Mazan in Katowice (Zbigniew Mazan Klinika Chirurgii Mazan in Katowice, ul. Brzozowa 54, NIP: 6252029012 – hereinafter: the “Clinic”).

Booking an appointment date for the procedure is made by arranging it at the Clinic’s registration desk – in person or by phone.

Booking a gastroscopy or colonoscopy appointment, to be effective, requires payment of a deposit in the amount of PLN 200. The deposit must be paid no later than 4 calendar days (i.e., 96 hours) before the scheduled appointment. If the deposit is not paid, the reserved appointment will be automatically cancelled.

The booking may be cancelled by the Patient without any financial consequences no later than 4 calendar days (i.e., 96 hours) before the scheduled date. Cancellation may be submitted by e-mail (to: sekretariat@klinika-mazan.pl) or by phone in the form of an SMS to the number: 691 476 095.

The deposit should be paid to the Clinic’s bank account, i.e.:

In the transfer title/description, it is mandatory to specify: “deposit towards an endoscopic procedure” – and if the payment is made from an account other than the Patient’s, also provide the Patient’s first and last name.

5. The paid deposit has the following meaning and will be settled as follows:

  • if the Patient undergoes the procedure on the agreed date, the deposit amount will be credited toward the price of the procedure charged in accordance with the Clinic’s current price list;
  • if the Patient does not undergo the procedure on the agreed date (fails to appear at the Clinic) or cancels the booking less than 4 days (i.e., 96 hours) before the procedure, the paid deposit is forfeited and retained by the Clinic;
  • if the booking is cancelled in accordance with point 3 above (i.e., no later than 4 calendar days (i.e., 96 hours) before the date), the deposit will be refunded to the Patient in full;
  • if, on the scheduled date, the procedure cannot be performed for reasons for which the Clinic is responsible – depending on the Patient’s decision – the deposit amount may be transferred toward the procedure on another date agreed by the Parties, and if no other date is agreed, it shall be refunded to the Patient in an amount equal to double the paid amount.

This document sets out the rules for booking and paying a deposit for an endoscopic examination applicable at Klinika Chirurgii Mazan in Katowice (Zbigniew Mazan Klinika Chirurgii Mazan in Katowice, ul. Brzozowa 54, NIP: 6252029012 – hereinafter: the “Clinic”).

Booking an appointment date for the procedure is made by arranging it at the Clinic’s registration desk – in person or by phone.

Booking a capsule endoscopy examination appointment, to be effective, requires payment of a deposit in the amount of PLN 500. The deposit must be paid no later than 5 business days (120 hours) before the scheduled appointment. If the deposit is not paid, the reserved appointment will be automatically cancelled.

The booking may be cancelled by the Patient without any financial consequences no later than 5 business days before the scheduled date. Cancellation may be submitted by e-mail (to: sekretariat@klinika-mazan.pl) or by phone in the form of an SMS to the number: 691 476 095.
